The web address where you talk about your coaching services. Include 'http://' at start.

Photo

A web address to a photo of you. It should be reasonably large and preferably include just you. Include 'http://' at start.

Contact

List all your contact info for prospective students. Alternate emails, telephone numbers, instant messenger accounts, etc.

Description

Describe yourself, your coaching experience, your philosophy, etc. Anything you want prospective students to know about you.

Please choose a password. Eventually you'll be able to edit your entry using your password. All passwords are stored in encrypted form.

Please check all that apply, and feel free to leave areas blank. For fees, just approximate if you don't charge hourly (i.e., are you "cheap" or "expensive"). For region, please check only those places you can meet with students face-to-face.
